Customized Diabetes Care Plan
Your diabetes care plan is not a one-size-fits all approach. You are unique in your food preferences and interactions with food: when you cook, how you shop, where you eat out. Your care plan will focus on your nutrition patterns, and help you identify where you may need to modify, add or change your eating plan. A diabetes care plan is not about eating less of the foods you love. In fact, it may surprise you that you can still enjoy many of the foods you currently eat. Long Term Support
Diabetes is a serious health condition and patients seeking support for diabetes should be prepared to work with our office for a minimum of 6 months. The initial visit will be 1.5 hours long, follow-up will be 30 mins to 1 hour in length depending on your specific needs. Your first follow-up will be 1-2 weeks after your initial appointment and then every 2-4 weeks for the first 3 months. Unlimited email and phone support is avaliable for all of our patients.
